AGREEMENT TO TREAT/PROVIDE LVT SERVICE
Service Cancellation- Please notify Sky Valley Pet Nurse LLC (SVPN ) of any cancellations or changes to your scheduled services 24 hours ahead of scheduled service. Failure to do so will incur a cancellation fee of half the scheduled service rate.
Cease Service Due to Pet Stress or Aggression- SVPN reserves the right to
cease service/treatment due to pet stress, aggression or potential injury at the discretion of
SVPN. If this is to occur, a cancellation fee of half the scheduled service rate will be due.
Medical Records- The Client authorizes SVPN to share their pet’s medical information/records with the referring veterinarian when and if applicable.
Rabies Vaccine Information- In the event of a traumatic injury to SVPN staff caused by your pet, your pet’s information may be disclosed to healthcare officials.
Restraint Policy- SVPN may use restraint at the discretion of the LVT working with
your pet. Forms of restraint used may be manual, muzzles, towel restraint or plastic cone collar.
Cease Service- SVPN reserves the right to cease treatment due to patient
stress, aggression or injury to themselves, other pets, or people. If this happens 50% of
service rate is due.
Payment Due- Payment is due in full within 24 hrs of time of service.
Prices- SVPN reserves the right to change fees and rates.
Rate Sheet- A copy of the Fees and Rates document will be provided to The Client upon request.
Travel Rates- SVPN charges a modest fee for traveling services outside of the Monroe/Snohomish city limits.
Company Policies- All services performed by a licensed Veterinary Technician. SVPN does not give out any medical advice or prescribe medications. Nursing care is only performed as recommended by a licensed veterinarian. The Client understands that SVPN will only perform nursing care as instructed by a veterinarian and will not be responsible for decisions regarding the pet's health made by the veterinarian and/or owner.
The Client agrees to notify SVPN of any signs of illness or injury to the pet prior
to scheduled service. If the threat of an infectious condition may exist, SVPN reserves the right to cancel services.
All services will be supervised by the client/pet owner or by an appointed guardian, with the exception of pet sitting services. The Client/ pet owner will be kept informed of the pet(s) wellbeing via phone, text or email. The client will provide all pertinent information in writing prior to the first day of pet sitting care.
SVPN always strives to provide the services agreed upon with the utmost care
and compassion and prevent accident or injury, however such things may occur no matter
how well the pet is cared for. The Client is responsible for all medical expenses and/or damages that are the result of an injury to the pet care provider by the client’s pet. The Client agrees to hold harmless,indemnify, and defend SVPN if a claim is made by any person injured by the client’s pet.
SVPN is not responsible for any damage to the client’s property caused by the
client’s pet.
The client agrees to not assert any claims against SVPN with connection of
treatment except those that involve gross negligence or willful misconduct on the part of
SVPN. The Client agrees to contact SVPN directly regarding anyconcerns within 24 hours of occurrence.
This agreement is valid for all future service dates without the need for an additional
contract to be signed with each future service. The Client agrees that the signed Agreement
to Treat contract will be valid for all future services for their pet(s).

Pet Sitting Service Agreement

Sky Valley Pet Nurse LLC
1. Sky Valley Pet Nurse (SVPN) agrees to provide pet sitting, dog walking and other
applicable pet care services in a reliable, caring and
trustworthy manner. In consideration of the services and as an express condition thereof, the
client expressly waives and releases SVPN from any and all claims against the company, its
owners, employees and representatives, except those arising from
negligence or willful misconduct on the part of SVPN.
2. Client agrees to notify SVPN of any concerns within 24 hours of completion of services.
3. Client agrees to pay all charges accrued for services rendered. Client understands that
payment is due at the completion of services unless explicitly agreed upon supplementary
payment terms.
4. SVPN accepts Cash, Venmo, PayPal and Zelle. Checks accepted from established
customers.
5. SVPN shall exercise all precautions against sickness, injury, escape, loss, accidents or death
of Client’s pet(s). SVPN is not responsible for sickness, injury, escape, loss accidents or death
of Client’s pet(s) unless caused through negligence or
willful misconduct on the part of SVPN.
6. Client represents and warrants that pet(s) are currently vaccinated in accordance with all
local, state and federal laws and regulations.
7. SVPN will follow the directions of the Veterinary Release Form in the case a pet should
become injured or sick.
8. Client accepts responsibility for all medical expenses and other damages resulting from an
injury to the SVPN, other persons or
other animal(s) caused by the Client’s pet(s) or negligent act.
9. Client agrees to indemnify, hold harmless and defend SVPN in the event of a claim by any
person injured or otherwise
damaged by Client’s pet(s) or negligent act.
10. SVPN reserves the right to charge a cancellation fee of 50% of the scheduled visits for
services canceled with less than
24 hours’ notice prior to the scheduled service.
11. SVPN reserves the right to terminate this contract at any time if the Pet Sitter, in his/her sole
discretion, determines that
the Client’s pet(s) poses a danger to the health or safety of itself, other pets, other people or the
Pet Sitter. If concerns prohibit the Pet
Sitter from caring for the pet, SVPN will attempt to contact to contact the Client to
arrange alternative care. If the Client cannot be contacted, the Client authorizes SVPN to place the pet in a licensed kennel with all charges and fees arising to
be the responsibility of the Client.
12. SVPN reserves the right to refuse service to any client, at any time, for any
reason.
13. This document gives SVPN and its representatives authorization to enter the Client’s listed
address as needed to perform agreed upon services.
14. Client expressly gives SVPN the authority to employ a locksmith on their behalf and to
promptly reimburse SVPN for all costs incurred in the event of a malfunction of the lock, keys or
automatic door opener.
15. SVPN is not liable for any loss or damage in the event of a burglary or other crime that
should occur while under this
contract.
16. Client agrees to properly secure the home prior to leaving the premises. SVPN will re-secure
the home to the best of its
ability at the end of each visit.
17. In the case of an emergency, inclement weather or a natural disaster, the Client authorizes
SVPN to use reasonable
judgment for the care and well being of Client’s pet(s) and residence. SVPN will make
reasonable efforts to maintain
service during these conditions but reserves the right to adjust the schedule of service based on
the sole discretion of the Pet Sitter.
18. SVPN is not responsible for any damages beyond the control of the Pet Sitter.
19. Client is responsible for supplying the necessary equipment and supplies needed for the
care of their pet(s) including, but not limited
to, a sturdy, well-fit harness or collar and leash for walks or in case of emergencies, pet food,
medications, identification tags, litter
boxes, cat litter and cleaning supplies. Client authorizes any purchase of necessary for the
satisfactory performance of duties. Costs of
all purchases and related service fees will be reimbursed to SVPN with 14 days.
20. Client authorizes the use of pet(s) pictures on website, social media and/or marketing
materials for promotional purposes.
21. Client authorizes this contract to be valid approval for services so as to permit SVPN to
accept all future in person,
telephone, online, mail or email reservations and provide services without additional signed
contracts or written authorizations.
22. The Term of this document apply to all pets owned by the client, including any and all new
pets that the customer obtains on or after
the date this document was signed/accepted.
